​​​​​​​

As part of Amplar’s Clinical Governance requirements, nurses must meet this experience level before completing their Amplar onboarding and getting started with HITH shifts.

🧑 My Home Hospital Orange Box

Most My Home Hospital patients will have an orange supply box in their home which may include equipment such as:
  • Blood pressure machine
  • Dressing supplies
 
    🩺 General Supplies
    Supplies are usually located:
    • In the patient’s home, or
    • In your nurse supply pack

    If supplies are missing:
    • Escalate to the Care Coordinator
    • Additional stock can be arranged for the client

    Amplar Health have a SharePoint site holds up to date resources including: 
    • Common system links and quick access tools
    • Nursing resources and clinical documentation
    • Clinical quick links for frequently used applications and information

    • Access to the Australian Injectable Drugs Handbook (AIDH), which is now only available through the SharePoint site

      All stock orders must be submitted via the Agency Kit Stock Ordering Form:
      Important Reminders
      Please keep an eye on your stock levels and plan ahead when placing re-orders.
      • Place orders at least 1 week in advance
      • Order processing takes approximately 2-3 business days
      • Shipping can take up to 3-4 business days
      • Nurses are responsible for managing and re-ordering their own kit stock
      • Stock held in the Adelaide Amplar Health office is for urgent needs only and availability is limited

      πŸš— Mileage Reporting

      If you've been approved to claim mileage for travel undertaken during your shift, all claims must be tracked and submitted Driversnote workplace called uPaged Team.

      Only work-related travel completed during your rostered shift is claimable. Mileage reports must be submitted by 5:00pm each Monday to be included in the Wednesday pay run.

      πŸ—ΊοΈ Adelaide Round Locations

      Hospital in the Home (HITH) shifts will now be posted using geographic zones across Adelaide. These zones align with Amplar Health's internal scheduling model and are designed to provide nurses with a clearer indication of the area where patient visits are expected to occur.

      Using zone-based locations helps nurses better understand the general area they are booking into and estimate travel requirements before accepting a shift.

      πŸš— Remember: If your commute to your first patient, or your journey home after your last patient, is more than 25 km, we'll reimburse the distance over 25 km. To claim this, simply let us know and add the additional reimbursable distance to your mileage report in Driversnote.​​​​​​​

      🎟️ Induction & Onboarding

      Community nursing requires you to be set up in numerous IT systems and trained in how to use them. Nurses are provided a 2 hour induction in the Amplar head office, followed by a buddy shift.

      πŸ‘©β€βš•οΈ A Day in the Life of an Amplar Community Nurse

      To help you feel prepared and confident before your first Amplar Health shift, we’ve outlined what a typical day may look like - from receiving your patient round through to completing your final visit. This guide explains what to expect, your responsibilities at each stage of the day, and how to respond when schedules change, supporting safe, efficient and consistent care for every patient.

      Before Your Shift
      • Your patient round is generally allocated and available to view from approximately 2:00PM on the day prior to your scheduled shift. Please contact Amplar Health, if you have not received it by 5:00PM.
      • Review your allocated visits as soon as they are published, you are not expected to review prior to your shift start time
      • Your shift start time is when you are expected at the first patient’s home
      • Check that all the visits align with your clinical skills, competencies, and scope of practice.
      • Confirm that you have the required equipment, consumables, and stock available to safely complete the planned care for each patient.

      The scheduling team will build your round in the most appropriate order to support efficient service delivery and minimize unnecessary travel.

      Planning Your Day
      • Review your schedule and familiarise yourself with patient locations, appointment times, and care requirements.
      • Confirm and accept your round for the day through Staff Appointments.
      • Review any clinical notes, care instructions, or updates before entering the home.
      • Escalate any concerns regarding visit allocation, clinical competency requirements, or stock availability as early as possible.

      During Your Shift
      • Travel to each patient as scheduled (your shift start time is when you are expected at the first patient) and deliver care in line with the care plan and Amplar clinical standards.
      • Upon arrival, confirm the visit as "Arrived" in the system.
      • Review the patient notes before entering the home.
      • Complete all documentation and handover activities at the end of each point-of-care service delivery- either in the home, or in your car before leaving to next patient.
      • Complete any required handovers or escalations.

      Changes to Your Round
      • Community nursing is a dynamic environment, and your schedule may change throughout the day due to:
        • Patient discharges
        • New referrals and urgent visits
        • Changes in patient clinical needs
        • Service delivery requirements
      • Any changes to your round will be communicated to you by the scheduling team.
      • You are responsible for regularly checking your round throughout the day and reviewing any newly allocated visits.
      • New visits can be reviewed and accepted through Staff Appointments.

      End of Your Shift
      • Ensure all visits have been completed and documentation is up to date.
      • Replenish stock and equipment where required, ready for future shifts.
      • Raise any operational, scheduling, or clinical concerns that may impact ongoing patient care.
